Ensuring health and well-being a collaborative effort

Ensuring the health and well-being of individuals supported by The Guild’s Adult Services is a collaborative effort. Each individual is assigned a nurse case manager, who works with the residential manager, house staff and family/guardian to ensure each resident receives the support needed to access medical and health care and maintain a healthy lifestyle.

Nurse case managers ensure that all medical appointments occur as scheduled, facilitates communication with health-care providers, and works with residential staff and families to ensure the implementation of all doctor’s orders and health-care recommendations.

Nurse case managers provide training to all residential staff on each individual’s health-care needs, including seizure protocols, specialized dietary plans and medical treatments, and also monitor the implementation of the individual health-care plan. Nurse case managers also train staff on supported and protective devices such as wheelchairs, ankle foot orthosis braces, splints and shower chairs.

Nurse case managers consult with the residential manager and staff in the implementation of the Massachusetts Department of Public Health Medication Administration Program (MAP). Medications are administered at each residence by MAP-certified staff. Additionally, residential staff are trained in first aid, cardiopulmonary resuscitation and the use of automated external defibrillators and EpiPens.
